This arrangement is a transcription of my 2016 recording of "The Church’s One Foundation" (hymn tune "Aurelia").
The first part of the piece is scored in 12/8 for simplicity of notation, but the feel is eighth-note triplets in 4/4. When the time signature switches to 4/4, keep the same beat going. In other words, the tempo of dotted-quarter notes in the 12/8 section = the tempo of quarter notes in the 4/4 section. To make the switch cleanly, it may help if you think of the fermata in measure 41 as the beginning of the 4/4 section. Hold that note for a solid 3 beats (as if it were a dotted half note in 4/4), with beat 4 of the implied 4/4 measure being the final note of measure 41, and come in with a strong quarter-note pulse in measure 42.
